The Choices Team includes a physician, nurse practitioner, nurse, social worker, chaplain and volunteers. All medical professionals on the team are certified in hospice and palliative care.

The Choices Team decides, along with the patient, who on the team will be involved in the care. For example, patients enrolled in the Choices program may not need to see a social worker or a nurse, or the nurse may just make phone calls to the patient rather than visits. The makeup of the team and what the patient receives from Choices is based on their needs and how we can best meet those needs.

Colleen Hazlett-O'Brien, DO, Director of In-Home Palliative Care

Education and Specialty:

Education: Philadelphia College of Osteopathic Medicine
Specialty: Hospice and Palliative Care, Family Medicine

Originally from Clarion, PA, Dr. Hazlett-O’Brien moved to Philadelphia to complete her undergraduate degree at the University of the Arts in musical theatre. After working as a professional actor in Philadelphia for years, she decided to change career paths and pursue a degree in medicine. She attended the Philadelphia College of Osteopathic Medicine, graduating in 2016. After graduation she and her husband, Michael, moved to Delaware where she completed her Residency in Family Medicine at Christiana Care Health System. After residency Dr. Hazlett-O’Brien completed a fellowship in Hospice and Palliative Medicine and graduated in July of 2020. Dr. Hazlett-OBrien has two boys and is thrilled to be back in Pennsylvania and grateful to Hospice & Community Care for the opportunity to pursue her passion in palliative medicine.

Krista LeCompte, NP

Education and Specialty:

Education: University of Pennsylvania and Harvard University
Specialty: Adult-Gerontology Acute Care, Hospice and Palliative Medicine

Krista graduated with honors from UPenn with a master’s degree in nursing and a minor in Palliative Medicine in 2013. She started her career as a nurse practitioner in Otolaryngology before transferring to Cooper University’s Palliative Medicine practice. While at Cooper University, Krista worked on the inpatient Palliative Medicine team and at the outpatient oncologic Palliative Medicine clinic for five years. She completed a post-master’s program at Harvard University- Palliative Care Education and Practice. Krista has been with Hospice & Community Care since 2018 and has held multiple roles including inpatient palliative medicine, inpatient hospice care, an embedded heart failure Palliative Medicine clinic, and a home-based Palliative Medicine program.

Steve Sensenich, RN, CHPN, Director of Specialty Services

Education and Specialty:

Education: St. Joseph Hospital School of Nursing and Millersville University
Specialty: Certified Hospice and Palliative Nurse

Steve has worked at Hospice & Community Care for more than 22 years, both in home hospice and at the Bob Fryer & Family Inpatient Center. Most recently, he was the Team Leader for the Supportive Care program. In March 2023, the Supportive Care program became Choices Palliative & Supportive Care at Home.
