What is Choices Healthcare?
Choices Healthcare is the parent company formed through the partnership of Hospice & Community Care (HCC) and Hospice of Central PA (HCP). While our trusted local names—HCC and HCP—remain in place for the communities we serve, we are now united under Choices Healthcare to expand access to exceptional hospice, palliative and grief support services across South Central Pennsylvania.
Will I still see HCC or HCP in my community?
Yes. Public-facing, you will continue to see Hospice & Community Care and Hospice of Central PA in your community. Behind the scenes, our staff are employed by Choices Healthcare, our Medicare number is under Choices Healthcare and your Explanation of Benefits (EOB) may list Choices Healthcare.
Who will provide my care?
When patients are referred to Choices Healthcare (HCC or HCP), they are cared for by our expert teams based in Harrisburg, Lancaster, Marlin, Mount Joy and York—close to where the patient calls home. No matter which office serves you, we are one team committed to providing the same high-quality, compassionate care we have been trusted to deliver for more than 40 years.
What programs are part of Choices Healthcare?
Under the Choices Healthcare umbrella, we offer:
- Choices Palliative Care – Specialized home-based medical care for relief from the symptoms and stress of serious illness. Care can be provided along with curative treatment.
- Hospice & Community Care and Hospice of Central PA – Hospice is specialized care for people who have weeks or months rather than years to live and who are coping with any serious, life-limiting illness. The team provides expert medical care, pain management and emotional and spiritual support.
- Sunflower Pediatric Program – Pediatric palliative, hospice and perinatal care for children and their families.
- Pathways Center for Grief & Loss and The Journey Program – The area’s most comprehensive grief support and services specifically designed to help adults, teens and children cope with the loss of someone important to them. Support is available to anyone in the community, not just for those whose loved one was cared for by Hospice.
Why was Choices Healthcare created?
The partnership between HCC and HCP allows us to share resources, strengthen services and reach more people in need. It ensures that more patients and families across South Central Pennsylvania can access expert, compassionate care—wherever they call home.
