How to Prepare for a Hospice Admission
It can be helpful to have the following information and items ready for the clinical staff at your admission:
- Any insurance cards including Medicare Part D information.
- A copy of the patient’s advance directives such as healthcare proxy or Do Not Resuscitate Order (DNR).
- Current medications in their bottles/containers.
- A list of any current medical equipment including current provider.
