When to Call Hospice - Hundreds of times each year, patients and families tell us: "We wish we'd entered hospice sooner." Patients and families can benefit most from hospice care when they seek support earlier rather than in a crisis. This page contains answers to questions such as signs of a patient's readiness for hospice care, physician involvement and benefits of receiving hospice care.
An Overview of Hospice Care - This overview describes the many types of illnesses that are helped by hospice care, a list of the team that is involved in patient and family care and where you can receive hospice care.
Hospice Care at Home - The majority of hospice care happens in patient's homes — providing medications, equipment and team members making regular visits, based on the patient's goals for their care.
Hospice Care in Nursing Facilities - Nearly half of the patients Hospice & Community Care cares for reside in Nursing Facilities, whether skilled nursing facilities, assisted living areas or continuous care retirement communities. The hospice team partners with the facility staff to provide the best palliative care possible for patients at each of the 31 local facilties in Lancaster County and several in neighboring counties.
Lancaster Hospice Center - If a patient is in need of very specific, hands-on care for crisis or other management of his or her pain or other difficult symptoms, our 12-bed facility on Good Drive in Lancaster presents a warm, home-like atmosphere for patients and their families.
Mount Joy Hospice Center - If a patient is in need of very specific, hands-on care for crisis or other management of his or her pain or other difficult symptoms, a second Inpatient Center in Mount Joy, features 24 beds and all of the same home-like touches including a meditation room, full kitchen. This section offers a family room and lovely gardens for patients and families to utilize during their stay.
