95 percent of our care is provided in the patient’s home. Our team of physicians, nurse practitioners, registered nurses, licensed practical nurses, hospice aides, social workers, counselors, chaplains and volunteers care for more than 400 patients and their families every day. Employees are trained and equipped with laptop computers, making the most recent patient information centralized and easily accessible.
